Setting Up Additional Questions

Schools have the ability to ask custom/additional questions as part of the Athletic Clearance process. Users can select the response types that their parents/students can input to limit responses received. Users will have the following response types:

Checkboxes - Provide responses where parents/students can select multiple responses. 

Dates - Allow parent/students to enter a date from a calendar widget.

Dropdown - Provide responses where parents/students can select a single response from a dropdown menu. 

Multiple Choice - Provide responses where parents/students can select a single response. 

Paragraph - Allow users to enter a text response in a large text field. 

Short Answer - Allow users to enter a text response in a small text field. 

To set-up Additional Questions:

• Click Add Question
• Type in Question
• Select Type of Response
  
  • For Checkboxes, Dropdown & Multiple Choice Questions enter in selections under Option.  After each selection hit ENTER.  The selection will show up in Green indicating it is a choice for users. 
• If the question needs further clarification, type in more information under Description
• Select Applicable sports.  If a sport is not selected the custom question will not show up on clearances for that sport.
• Choose to make the question a requirement for completion.

Example of Additional Questions that can help you gather information about your Athletic:

• Helmet Size
• Jersey Number
• Shoe Size
• Preferred Position
• Drivers License Numbers
